Microcatheter-assisted circumferential trabeculotomy
Illuminated microcatheter-assisted circumferential trabeculotomy for childhood glaucomayous eye was performed. Bulbar conjunctiva was cut parallel to corneal limbus at 12:00 meridian. Lamellar scleral flap was made with the thickness of 1/2 sclera and size of 4 mm × 4 mm by a 1.25 mm tunnel knife. A deep sclera incision across scleral process to Schlemm tube was carried out at 0.5 mm inside the shallow scleral incision under the lamellar scleral flap. Illuminated microcatheter was used to insert the Schlemm tube for circumferential 360º and than pulled the microcatheter to cut the wall of Schlemm tube. The bulbar conjunctiva was closed with 8-0 absorbable sutures.
